Job Details Job Location: Sunrise - Fresno, CA 93705 Position Type: Full Time Salary Range: $27.10 - $37.48 Hourly JOB SUMMARY The ACT/FACT Nurse reports to the Regional Nursing Supervisor and the Program Director and works as a part of the ACT/FACT treatment team in achieving the goals and objectives of the program to ensure quality of care for client participants. ACT/FACT is a community-based, team-based service to support members living with complex and significant behavioral health needs and a treatment history that may include psychiatric hospitalization and emergency room visits, residential treatment, involvement with the criminal justice system, homelessness, and/or lack of engagement with traditional outpatient services. ESSENTIAL JOB RESPONSIBILITIES Provide high-quality nursing services to clients living with Severe Mental Illness (SMI) and dual diagnosis. Services include but are not limited to medication education and support (scheduled, walk-in, individual, groups, and crisis intervention), consultation with Psychiatrist and physicians regarding clients and medication adjustments, facilitation of blood draw for labs as ordered, assistance with medical treatments/procedures as prescribed. Per authority of license, administer medications to patients, chart appropriate records, review medicine orders regularly, obtain appropriate physician approval, and coordinate with pharmacy as need. Plan, coordinate, and facilitate services for Severely Mental Ill (SMI) clients. Responsible for the monitoring and observation of client medications; administration of prescribed injections; and tracking of prescribed medication and dispensing as appropriate. Provide frequent and assertive contact to individuals, primarily community/ field-based services with occasional office-based services. Order supplies and check delivery; stock and maintain inventory. Assume responsibility for general client care and assist with medical treatment. Transport clients to doctors’ appointments and laboratories for tests (as needed). Maintain appropriate case recording documentation as per the Rehab option Medi-Cal guidelines, and other contract guidelines. Work on a multi-disciplinary team to support clients with an array of needs delivered as an integrated service, including, but not limited to, medications, therapy, and groups, housing, employment, benefits, transportation, physical health, and substance use. Be available to respond to crisis in the community. Conduct individual medication education, crisis intervention and pre-care coverage. Consult with physicians regarding patients and medication adjustments. Adhere to reporting requirements for incidents of Child Abuse, Elder Abuse, Adult Abuse, Dependent Adult Abuse, and Tarasoff warnings per California reporting laws. Meet monthly key performance metrics, including end of the month deadlines, productivity standards and documentation timeliness expectations. Communicate closely, in a systematic fashion, with agencies and significant others involved in the rehabilitation of clients. Visit clients in their homes, board and care facilities, or in other community placement locations to carry out prescribed treatment plans. Observe client’s behavior and physical condition, report significant changes to treatment team, and document client progress. Support psychiatrist or nurse practitioner with capturing and recording vitals, allergies, and medication allergies. Assist with administration of medications, including hypodermic injections. Coordinate with pharmacies to obtain and/or deliver medications for clients and provide medication education. Prepare and maintain a variety of records and prepare reports including documenting services to meet all clinical program and state requirements, including Medi-Cal/ CalAIM requirements. Attend daily team check-in meetings, weekly one-on-one meetings with Supervisor, and critical bi-monthly meetings including staff meetings, and high risk resource team (HRRT) meetings. Provide effective and thorough feedback regarding client health and medication engagement, progress, barriers, and needs Think and act quickly and efficiently in emergencies. Complete training requirements set by the agency and county. Will drive on agency business, either in a company vehicle or your personal car, including picking up, transporting and dropping off clients. Physical presence at the program is required. ESSENTIAL JOB REQUIREMENTS Possession of a valid license as a Registered Nurse, Psychiatric Technician or Licensed Vocational Nurse as issued by the State of California. Supplemental college course work in behavioral or social sciences, and one year of experience performing duties comparable to a Mental Health Worker or related nursing experience, preferred. Knowledge of basic concepts of mental illness and behavioral health including the physical, emotional and social needs of individuals living with severe mental illness. Certified training in emergency first aid procedures including C.P.R, preferred. Knowledge of fundamentals of psychiatric and mental health nursing care. Knowledge of basic medical and psychiatric terminology. Knowledge of principles and techniques involved in the care and treatment of individuals or groups of mentally ill, emotionally disturbed, severe mental illness (SMI), dual-diagnosis individuals. Knowledge of contra-indications, side effects and signs of medication complications. Ability to apply routine psychiatric/medical nursing care to client with Severe Mental Illness (SMI) and Dual-Diagnosis. Recognize symptoms indicative of adverse patient reactions. Knowledge of record-keeping procedures related to client charts. Analyze situations accurately and take effective action. Participate in treatment programs for patients. Understand and follow written and oral directions. Ability to communicate effectively in written and spoken English Establish and maintain effective working relationships with others. Work independently in proving coverage at outreach locations or providing patient supervision assistance. Ability to operate within Electronic Medical Records (EMR) system. Basic to Intermediate computer skills in Office 365, Teams, Zoom and other communication/meeting platforms. Possess a valid driver’s license and have access to a dependable means of transportation that is properly insured and operated in accordance with all laws. Agency insurer authorization is required. Ability to pass a pre-employment criminal background check. Ability to pass a pre-employment drug screen and TB test. ESSENTIAL JOB REQUIREMENTS FOR BILINGUAL SKILL Communicate effectively in written and spoken Spanish. Required ability to communicate effectively with monolingual Spanish speaking clients and families for all clinical purposes. Assist with phone calls for Spanish speaking clients. Required to provide language translations for case managers and Physicians on a periodic basis. Minimal adequate competency in the Spanish language as evidenced by successful passage of the TPOCC Spanish Competency Test. Other translation tasks as required.
