Home Health Physical Therapist
Location: East Portland, OR and Surrounding Areas
Assignment Duration: This is a 13-week contract position.
Schedule: Day Shift: 5x8-Hour (08:00 - 16:30)
Job Description: We are seeking a dedicated and experienced Home Health Physical Therapist to provide personalized care to patients in their homes in East Portland and surrounding areas. The ideal candidate will demonstrate a strong commitment to providing high-quality therapy while managing patient needs efficiently.
Responsibilities
- Evaluate and develop personalized treatment plans for patients.
- Conduct home visits, adhering to productivity standards.
- Provide skilled physical therapy treatments and interventions.
- Document patient progress and adjust treatments as necessary utilizing EPIC and Oasis systems.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.
Required Experience / Certifications / Licensure
- Minimum of 1 year of skilled home health experience.
- Valid Physical Therapist license as per state regulations.
- Basic Life Support Certification (BCLS) required.
- Oasis experience is required.
