Behavioral Health Care Manager

BH Care Manager ~ MSW, LMSW, LCSWThe BH Care Manager within the integrated primary care and behavioral health team is responsible for providing care management for patients of FCHC and for clinical supervision of non-licensed care managers. The role includes demonstrating a commitment to FCHC mission and vision, a positive attitude towards patients, employees, role, and the health center, and adherence to FCHC core values. The position requires customer service orientation, timely response to requests, good communication skills, conflict resolution skills, and adherence to FCHC's dress code policies. The BH Care Manager must consistently show commitment to position and team performance, consideration and acceptance of cultural differences, and participation in training and professional development. Safety adherence, including HIPPA/Confidentiality standards and respectful use of Health Center Equipment, is also essential.The BH Care Manager functions within the Behavioral Health Services department, reporting to the Manager of BH Care Management. The primary responsibilities include enrolling patients into the Circle of Care project, tracking patient progress, conducting face-to-face visits and telephone calls, establishing and maintaining a patient registry, arranging for inpatient and outpatient programs, coordinating information flow between providers, attending care team meetings, identifying treatment barriers, establishing referral relationships, facilitating referrals to other therapists and psychiatry, participating in staff in-service programs, and providing clinical supervision for LMSWs (for LCSWs only). Periodic duties include contributing to community public health activities, participating in Center work teams, and performing other assigned duties.Qualifications for the BH Care Manager include an MSW, with LMSW and LCSW preferred. A minimum of three years of experience in social work or behavioral health care is required, with work experience or strong desire to work in an urban community health setting preferred. Active professional development and maintenance of continuing education standards as established by the Division of Professional Registration of Missouri are strongly recommended for LMSW and LCSW. Family Care Health Centers
