This role allows for remote work, providing a unique opportunity for flexibility. You will be part of a multidisciplinary team consisting of healthcare professionals, including nurses, physicians, and spiritual care providers, who collectively focus on the psychological and emotional needs of our patients. Key Responsibilities\n

  • Conduct thorough mental health assessments to evaluate the emotional and psychological needs of patients diagnosed with terminal illnesses, taking into consideration their medical history and individual needs.

  • Collaborate with other healthcare professionals to create and implement personalized treatment plans that focus on symptom management, emotional well-being, and the integration of mental health care into the overall hospice plan.

  • Deliver evidence-based interventions, including individual and family counseling, and therapeutic modalities such as cognitive-behavioral therapy (CBT) and solutions-focused therapy to help patients and their families cope with grief, anxiety, and adjustment challenges.

  • Facilitate group therapy sessions for patients and families, fostering a constructive atmosphere for sharing experiences, coping strategies, and emotional support.

  • Regularly provide education to families on mental health resources, support groups, and coping strategies to ensure ongoing emotional support outside of therapy sessions.

Patient Population / Client Focus\n

This role primarily focuses on patients in hospice care who are confronting chronic illness and the end-of-life process. You will engage with patients from diverse backgrounds and varying degrees of mental health needs, ensuring that all individuals receive compassionate support tailored to their unique situations. Your work will directly impact their emotional well-being and that of their families.

Qualifications\n

To be successful in this role, candidates must hold a valid LCSW license and possess a Bachelor’s degree in Social Work or a related field. Strong communication and interpersonal skills are essential for fostering effective relationships with patients, families, and multidisciplinary team members.

Preferred Experience\n

While a minimum of three years of experience in mental health or clinical social work is preferred, candidates with a background in hospice care, palliative care, or similar environments will be highly regarded. Experience with trauma-informed care and bereavement counseling will strengthen your application.

Compensation and Benefits\n
