Schedule : Full Time | 40 hours per week | 8:00am - 4:30pm | Weekend & Holiday Rotations required

What You'll Do:

As a Lead Certified Nurse Aide (CNA) in our Day Center, you'll lead the CNA team and play a vital role in ensuring our participants receive the highest quality care. From assisting with daily living activities to training new hires, you'll bring confident leadership to Thome PACE.


Key Responsibilities

• Provide Hands-On Care - Assist with personal care (ADLS) like bathing, grooming, dressing, and toileting.
• Lead with Confidence - Mentor, train, and support CNAs by overseeing onboarding, scheduling, and professional development to ensure quality participant care and adherence to PACE values and policies.
• Support Mealtime- Help participants enjoy their meals and stay hydrated for good health.
• Keep Things Moving - Support mobility, including turning, positioning, and safe transfers.
• Perform Vital Checks - Measure and record vital signs, weight, and intake/output.
• Maintain Comfort & Care - Keep rooms tidy, cozy, and well-stocked with essentials.
• Provide Responsive Support - Answer call lights quickly and respond with kindness and compassion.
• Monitor with Watchful Eyes - Report any changes in conditions to the leadership ASAP.
• Demonstrate Respect & Professionalism - Maintain confidentiality while delivering top-notch care.

On-Call and Flexibility:

• On-Call Responsibilities: Participate in after-hour on-call rotations, including evenings, weekends, and holidays.
• Rotation Schedule: Be available for evening and weekend shifts as part of the rotation.

Skills, Knowledge and Expertise

• A current and valid Michigan CNA Certification, required.
• CPR Certified or willing to obtain.
• Two (2) years of clinical experience as a licensed CNA.
• The ability to read, write, understand, and follow directions.
• A heart for helping others and the physical ability to lift, stand, bend, transfer, stretch, walk, push, and pull as needed.
• Ability to lift a minimum of 35 pounds frequently is required
• Must be 18 years of age or older.
