Conviva Senior Primary Care, part of Humana's Primary Care Organization, seeks a Nurse Practitioner to provide health promotion and treatment for seniors. You will diagnose illnesses, prescribe medications, and work with a multidisciplinary team to support holistic wellbeing. Reporting to the Associate Medical Director for the Corpus Christi market, you will float among clinics regionally, maintain documentation, and participate in the value-based care model with full-time hours (40/week). #J-18808-Ljbffr
