Immediate Opening – Start March 12, 2026
Charity Care Group is seeking a Licensed Practical Nurse (LPN) to provide one-on-one skilled nursing care for a 29-year-old male with a G-Tube attending a Suffolk Day Support Program .
This is a daytime weekday position with consistent hours and the opportunity to provide focused patient care in a supportive environment.
Schedule: Monday – Friday
Hours: 9:00 AM – 4:00 PM
Status: PRN (3–5 days per week)
Meet & Greets are being held daily at 11:00 AM starting tomorrow.
ResponsibilitiesProvide skilled nursing care during day support hours
Manage G-Tube feedings and medication administration according to the care plan
Monitor patient condition and report any changes
Maintain accurate nursing documentation
Work collaboratively with day support staff and family members
Active LPN license in Virginia
Current CPR certification
G-Tube experience preferred
Experience with individuals with developmental or intellectual disabilities is a plus
