Job Responsibilities Utilize the nursing process to care for patients, considering patient age, cultural, and psychosocial factors. Document steps of the nursing process using proper medical terminology; address problems and progress toward goals. Facilitate patient discharge through timely, appropriate referrals and utilization of patient education resources, suitable to patient age and cultural/psychosocial factors. Collaborate with other disciplines as appropriate; support unit/hospital goals through PI and educational activities by attending in-services, meeting unit education standards, and completing variance reports. Decision Making Exercise independent judgment in making decisions from many diversified alternatives that are subject to general review in final stages only. Working Relationship Coordinate activities of others (does not supervise) and lead others in the same work performed (does not supervise). Provide input on performance evaluations but do not prepare or give them. Work directly with patients and/or customers, as well as internal and external customers via telephone or face-to-face interaction. Collaborate with other healthcare professionals and staff, frequently interacting with individuals at Director level or above. Required Skills & Experience Up to six (6) months of related experience. Working knowledge of general disease state management processes, bio-psychosocial model, and clinical pathways. Effective interpersonal, written, and oral communication skills. Ability to organize and prioritize work in an effective and efficient manner. Detail-oriented as required in the examination of clinical and numerical data. Schedule: Nights 7p-7a, 2 x12 Relocation assistance provided for those moving from 71 miles No sign-on bonus.
