PRN RN Case Manager, Home Health

Good Samaritan is seeking a PRN RN Case Manager, Home Health to provide holistic, patient-centered care to underserved adults in Atlanta. This role conducts in-home assessments, develops and monitors individualized care plans, coordinates services with an interdisciplinary team, and educates patients and families on disease management and safety. The RN ensures regulatory compliance, accurate documentation, and timely communication while practicing in a faith-inspired, mission-driven environment focused on health equity and whole-person wellbeing.
