Job Title: RN Home Health Address: Orange Park, FL Job Description Administer skilled nursing care for clients of all ages in their place of residence. Assess home health and/or hospice patients in stable and emergent situations. Implement, develop, document, and evaluate the Plan of Care (POC). Educate patients and/or their families regarding care, diet, medications, and home safety. Supervise and provide clinical directions to healthcare assistants. Monitor assigned cases to ensure compliance with third-party payer requirements. Provide effective communication and serve as a liaison to patients, families, and team members. Critical Information Shift: M-F until shift is complete; On-call required if requested. Patient Service Area: Local travel required; reliable transportation needed. Education / Licenses Needed Current state license as a Registered Nurse. Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program. CHPN certification preferred.
