Key Responsibilities
As a Licensed Clinical Social Worker, you will be responsible for the following:
- Conduct comprehensive evaluations and assessments to understand the needs of the patient and their families.
- Participate actively in the development and implementation of Individualized Family Service Plans (IFSPs), ensuring that interventions align with the family's unique needs and preferences.
- Monitor and document patient outcomes as part of a multidisciplinary team, adjusting plans as necessary to optimize care.
- Provide empathetic support and counseling to families navigating the challenges of end-of-life care, enhancing their coping strategies and emotional resilience.
- Facilitate communication and collaboration between families and the care team, ensuring that all parties are informed and involved in care decisions.
- Adhere strictly to all Federal and State regulations while demonstrating compliance within your practice.
- Utilize evidence-based practices to recommend strategies that will foster the child's development, utilizing the Natural Learning Environment Practices.
- Ensure the health and safety of all individuals served and adhere to organizational standards.
Patient Population / Client Focus
In this role, you will primarily focus on patients receiving hospice care, which often involves elderly individuals with terminal illnesses. You will also interact with families, helping them navigate complex emotions and decisions during this critical time. Your efforts will be instrumental in shaping the quality of their overall care experience and ensuring they receive comprehensive support tailored to their needs.
Qualifications
To be successful in this role, candidates must possess the following:
- A Master's Degree or Doctorate in Social Work.
- Licensure by the Department of Public Health as a Licensed Clinical Social Worker (LCSW) is required.
- A valid driver's license is necessary to facilitate any required in-person visits.
- Strong communication skills to effectively engage with patients and families from diverse backgrounds.
- Exceptional organizational and time management skills to juggle multiple cases and responsibilities.
Preferred Experience
We prefer candidates who have:
- One or more years of experience working with children in early intervention.
- A demonstrated commitment to Natural Learning Environment Practices.
- Bilingual skills to better serve our diverse patient population.
- A proven ability to adapt their approach to various learning styles and needs to enhance effective treatment delivery.
Compensation and Benefits
This full-time position offers a competitive salary range of USD 75,000 – 80,000 annually, reflecting your expertise and commitment to care. Further benefits include:
