Care Coordinator

Hartford Healthcare is seeking a Care Coordinator RN to support patients across our Connecticut network. In this role, you will coordinate care plans, manage transitions between settings, and collaborate with physicians, nurses, and community providers to ensure safe, timely, and high-quality care. Responsibilities include assessing patient needs, arranging services, providing education, and monitoring progress. You will use electronic health records and clinical protocols to reduce readmissions and improve outcomes while supporting our culture of teamwork, compassion, and continuous improvement.
