Overview

Rock Creek Integrated Solutions is adding Physical Therapists to its PRN team, serving patients in hospice, home-based, and clinical settings. This role can be done fully remote through telehealth. Some of the patients we serve are in the Rapid City, South Dakota area, so the ability to see patients in person there is a big plus. Scheduling is flexible and pay is per visit.

Goals range from restoring mobility to keeping patients safe and comfortable at home, so you'll adapt each plan of care to what matters most to the patient and family.

Pay: $70+ per visit (PRN contract)

Duties

• Evaluate mobility, strength, balance, and fall risk via telehealth and, when possible, in person
• Build plans of care that match each patient's goals
• Recommend DME such as walkers, wheelchairs, and hospital beds
• Train caregivers on safe transfers, positioning, and fall prevention
• Communicate changes in patient status to the care team
• Document visits accurately and on time
• See patients in homes and facilities around Rapid City when available (a plus, not required)

Experience

• Doctor of Physical Therapy (DPT) or equivalent
• Active South Dakota PT license, or able to obtain one
• Adult or geriatric experience; home health, SNF, or hospice preferred
• Comfortable balancing patient safety with patient wishes
• Comfortable guiding patients and caregivers through exercises and transfers over telehealth
• Ability to see patients in person in the Rapid City area is a plus
