Compliance document deadline is Monday prior to start. Permanent and per diem candidates cannot return to Cleveland Clinic hospitals for 1 year from their previous end date. All RTO must be presented at time of submission only- facility will not review RTO submitted after this time.

2 references from a direct supervisor/manager, ANM, director for the unit submitted to 18 months experience Required, 2 years exp. preferred. Locals accepted at same BR Facility does not accept any license hits. Smoke Free Facility: All travelers MUST BE NICOTINE FREE- No Exceptions! First time travelers accepted No Certs pending EKG and Pharm test required during orientation. 2 attempts max for each. No remediation for a re-test. Travelers will be cancelled if they fail. Study materials provided with FDI Facility has a strict DO NOT call policy Modules: Modules are non-billable, average 6-15 hours Modules are completed pre-start and annually, and vary by specialty Modules are recorded via SilkRoad
