Job Details:


• Work Monday through Friday, approximately 7:45 AM to 4:00 PM
• See 6 patients/points per day (evaluation visits count as 1.5 points, standard visits count as 1 point)
• No weekend rotation, on-call, or holiday requirements
• Coverage area includes Racine and Kenosha Counties in Wisconsin
• Most patients within 20-30 minutes travel time, occasional patients up to one hour away
• Participate in morning team huddle via phone daily
• Work with interdisciplinary team including RN and PT professionals
• Provide general OT treatment and evaluation skills focused on ADLs and adaptive equipment
• Treat adult patients in home health setting including geriatrics, post-stroke, post-joint replacement, weakness, deconditioning, COPD, and CHF patients
• First day orientation at National Avenue office in Milwaukee, remainder of week with preceptor
• Use EPIC EMR system (preferred experience)
• Dress code: Business casual or any color scrubs
• Required: BLS certification, Wisconsin OT license, bachelor's degree in occupational therapy, valid driver's license, active auto insurance, and personal vehicle
• Minimum 3 years OT experience required (SNF, home health, or acute care preferred, with home health experience most preferred)
• First-time travelers accepted
