Job Function:

• Provide high-quality nursing care to participants in the Adult Day Center.
• Conduct assessments, monitor chronic conditions, and document clinical findings.
• Administer medications and treatments in accordance with care plans and organizational protocols.
• Collaborate with interdisciplinary team members including social workers, activity staff, and caregivers.
• Support care coordination between the Adult Day Center, families, and external healthcare providers.
• Respond to participant health concerns and emergencies promptly and professionally.
• Participate in quality improvement initiatives and mandated trainings.
• Maintain accurate electronic and paper medical records.

Job Requirements:

Job Requirements

• Active NY Registered Nurse (RN) License in good standing.
• Experience with geriatric, adult day programs, senior living, or related settings preferred.
• Strong assessment, communication, and organizational skills.
• Ability to work Monday-Friday, 8:00 AM-4:00 PM, with flexibility for 1 Saturday per month.
• Compassionate, patient-centered approach and a strong commitment to quality care.
