Shift: 8 hours

Assignment Details:

• Contract Length: 13 weeks
• Guaranteed Hours: 40

Requirements:

• Current Nursing License in the practicing state
• Valid Driver's License
• Current CPR Certification
• Two years of experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting
• Knowledge of electronic medical charting (Homecare Homebase)

Roles & Responsibilities:

• Administer skilled nursing care to patients at their place of residence as prescribed by the physician.
• Assist in identifying patient's physical, psycho-social, and environmental needs.
• Participate in planning, coordinating, and implementing care in conjunction with the Registered Nurse.
• Provide effective communication to patients/families, co-workers, and other healthcare professionals.
• Assist patients and families in learning self-care techniques and assess responses to care.
• Ensure care delivery adheres to infection control measures and complies with state and federal regulations.
