RN Care Coordinator: Ambulatory Hypertension Management

UC Health Psychiatry

UC Health is seeking an RN Care Coordinator for the Ambulatory Hypertension Management Program in Cincinnati. You will partner with primary care providers, specialists, pharmacists, and population health teams to improve blood pressure control, reduce cardiovascular risk, and meet value-based care targets. You will educate, monitor, outreach, and coordinate care for patients with uncontrolled hypertension. Ideal candidates hold an Associate's Degree in Nursing (RN) with Ohio licensure and 2 #J-18808-Ljbffr
