Responsibilities: Provide high-quality preventative care to residents in skilled nursing facilities through on-site visits and remote work from home Conduct assessments, develop treatment plans, and prescribe medications as needed Collaborate with healthcare teams to ensure comprehensive care and minimize hospitalizations Utilize RPM, CCM, and CoCM to enhance patient outcomes Maintain accurate and timely documentation of patient care and outcomes Participate in quality improvement initiatives to enhance patient care standards Requirements: Active NP license (in the state of practice) 1+ year of experience in Geriatric care and documentation review Excellent communication, collaboration, and teamwork skills Benefits: Hybrid role with flexible scheduling (8 hours per week) Support and Training from our clinical directors, regional managers, and supervisors Generous Paid Time off Competitive annual salary range of $20,000 - $22,000 #J-18808-Ljbffr Carease Health
