Registered Nurse – Home Care

Monday to Friday, 8 AM to 4 PM, with some weekends and on-call duties required.

Assignment Details

Contract length and guaranteed hours will be discussed during the interview process.

Requirements

• Current nursing license in the state
• A minimum of 2 years of nursing experience
• Knowledge of OASIS documentation requirements
• High-tech nursing skills, such as IV use and maintenance, wound care
• Ability to work collaboratively with LPNs and interdisciplinary teams

Responsibilities

• Conduct patient assessments and develop realistic care plans.
• Manage case loads and ensure effective communication with patients and care teams.
• Implement medical plans of care and evaluate patient progress.
• Perform patient and caregiver education on treatment options and care instructions.
• Document all care provided accurately and in a timely manner.
• Assist with safe patient discharges and coordinate care among various health professionals.
