• Conduct comprehensive psychosocial assessments through patient interviews, observations, and thorough reviews of medical records to evaluate emotional, social, and environmental factors affecting patients' well-being.

• Provide empathetic counseling and emotional support to patients and their families, helping them address adjustment issues related to illness, grief, and end-of-life considerations.

• Collaborate with an interdisciplinary team, including nurses, physicians, and therapists, to develop customized care plans that reflect the unique psychosocial needs of each patient.

• Assist patients and families in navigating community resources, including access to support groups, vocational rehabilitation services, and practical assistance such as housing and transportation.

• Actively participate in discharge planning to facilitate successful transitions from our hospice care services back to home or other care environments.

• Offer educational resources and workshops to families about coping strategies, community involvement, and post-rehabilitation services to empower them during this challenging time.

The primary clientele served by our hospice center includes individuals with terminal illnesses, complex chronic conditions, and their respective families. Our focus is on addressing the multitude of psychosocial needs that arise during the end-of-life process, thereby fostering a supportive environment that prioritizes comfort and dignity. Each patient's unique background and experiences are honored, ensuring that care remains personalized and respectful.

• A Master’s degree in Social Work from an accredited institution.

• Active and valid state licensure as a Licensed Clinical Social Worker (LCSW).

• A strong understanding of adjustment issues, grief processes, and coping mechanisms related to chronic and terminal illnesses.

• Exceptional communication and interpersonal skills, with the ability to build trust and rapport with diverse patient populations.

• Prior experience in hospice, palliative care, or rehabilitation settings is strongly preferred.
