divh2Licensed Practical/Vocational Nurse/h2pThe Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes./ppEssential Job Functions/Responsibilities:/pulliProvides direct patient care as defined in the state Nurse Practice Act./liliImplements plan of care initiated by the registered nurse./liliProvides accurate and timely documentation consistent with the plan of care./liliAssesses and provides patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care./liliParticipates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Manager./liliUses and/or prepares equipment and supplies effectively and efficiently adhering to aseptic technique./liliParticipates in personal and professional growth and development./liliPerforms other duties as assigned by the registered nurse./li/ulpJob Qualifications:/pulliGraduate of an accredited practical nurse or vocational nursing program./liliHas three (3) years nursing experience. Community/home health or medical/surgical experience is preferred./liliCurrently licensed as a licensed practical nurse or licensed vocational nurse in the state./liliPossess and maintain valid CPR certification./liliComplies with accepted professional standards and practice./liliDemonstrates good verbal and written communication, and organization skills./liliMust possess a valid California Drivers License with automobile that is in good working order and insured in accordance with the organization requirements./li/ulpPhysical Requirements:/pulliThe ability to sit occasionally for 1-3 hours./liliThe ability to frequently stand for 3-6 hours./liliThe ability to occasionally drive/ operate foot control for 1-3 hours./liliThe ability to walk intermittently on level and or unlevel surface for less than 1 hour./liliThe ability to intermittently bend or climb for less than 1 hour./liliThe ability to occasionally twist, kneel, crouch and crawl for 1-3 hours./liliThe ability to lift 50 pounds frequently for 3-6 hours./liliThe ability to lift greater than 50 pounds occasionally for 1-3 hours./liliThe ability to carry up to 50 pounds frequently for 3-6 hours./liliThe ability to carry greater than 50 pounds occasionally for 1-3 hours./liliThe ability to push up to 50 pounds frequently for 3-6 hours./liliThe ability to push greater than 50 pounds occasionally for 1-3 hours./liliThe ability to pull up to 50 pounds frequently for 3-6 hours./liliThe ability to pull greater than 50 pounds occasionally for 1-3 hours./li/ul/div
