Full Time Professional Essex Junction, VT, US 7 days ago Requisition ID: 1585 Salary Range: $117,268.00 To $143,328.00 Annually CHC is looking to add either a Nurse Practitioner or Physician Assistant to our team. Nurse Practitioners and Physician Assistants (NP/PA) are clinical leaders responsible for providing direct outpatient medical services to CHC patients in support of the organizational mission. They render services that reflect current medical standards appropriate to the local community. Responsibilities include coordination and documentation of continuity of care, risk management, participation in quality assurance and improvement, lifelong learning, and supporting the financial stability of the organization. Basic Qualifications Graduation from a program of study accredited by the appropriate professional board. Ability to work independently and solve problems creatively with initiative. Affinity for working with a culturally and economically diverse patient population. Good organizational and interpersonal skills. Willingness to work flexible hours. Licensure, Certification or Other Requirements Licensure for practice in the State of Vermont. Board certified or eligible for board certification. Active DEA license. CPR certification. Completion of requirements for successful credentialing and privileging by CHC and applicable insurance companies. Essential Duties A. Direct Patient Care & Services Provide direct outpatient medical services within the scope of board certification (family medicine, general medicine, mental health, gynecology, family planning, pediatrics, geriatric medicine). Ensure medical, laboratory, and diagnostic follow‑up and timely EHR documentation in accordance with CHC policies. Maintain access to care by meeting productivity expectations and increasing panel size when possible. Communicate effectively and promptly with patients about health & illness, empower patients to take an active role in health maintenance, and promote wellness behaviors. Complete all clinical documentation promptly and in alignment with CHC policies. Refer patients to specialty care and ensure appropriate communication and follow‑up. Participate in CHC 24‑hour on‑call coverage and cover clinical responsibilities for colleagues not on duty. Follow CHC clinical policies and procedures. B. Clinical Supervision Seek out clinical supervision when needed, including case consultation to craft care plans and internal consultation with a supervising CHC provider. Provide clinical supervision for nursing staff as assigned for triage, medication refills, referrals, and nursing service visits. Provide teaching to trainees (medical, nursing, nurse practitioner, physician assistant students and resident physicians) as assigned. C. Quality Assurance/Quality Improvement Participate in quality assurance activities such as development of flow sheets and data collection instruments, chart audits, and case conferences. Participate in interdepartmental quality improvement team efforts. Participate in efforts to improve quality outcomes defined by non‑CHC entities (UDS, PCMH, Accountable Care Organizations). Contribute to CHC efforts to achieve higher‑quality and cost‑effective care for patients and improved quality of work‑life for staff. D. Continuing Education Participate in continuing education to meet licensure and board certification requirements; submit documentation to supervisor and credentialing files. Develop and present continuing education sessions for CHC staff members. Attend staff and team meetings as assigned or applicable. E. Community Presence Communicate a positive image of CHC to and in the community. Physical Demands and Working Conditions Work performed in an environmentally controlled office/health clinic setting.
