Job Title: Healthcare Consultant II - Jefferson County KY Address: Louisville, KY Job Description Supports comprehensive coordination of medical services, including care team intake, screening, and implementing care plans. Identifies patients' medical needs, makes referrals, and facilitates appropriate service delivery in line with benefit plans. Monitors cases for discharge dates, outliers, and triggers for specialized program referrals. Uses healthcare systems to research and enter member information; supports care plan development. Coordinates healthcare service delivery, promoting communication among providers and team members.
Critical Information
Remote, Work From Home - Kentucky residents only. Work hours: Monday to Friday, 8:00 AM - 5:00 PM EST, with flexibility until 6:00 PM EST. Required travel: 5% - 10% to Louisville office for meetings and training. Must have wired internet with minimum 25 Mbps download and 3 Mbps upload speeds. Quiet, secure home workspace in compliance with HIPAA guidelines. Utilizes Microsoft Office 365, Teams, Outlook, and secure system access. Ability to troubleshoot common technical issues and adapt to future workplace changes.
Education/Licenses Needed
High School Diploma or G.E.D. Experience or knowledge in foster care, juvenile justice systems, or behavioral health services is preferred. Computer literacy with proficiency in MS Office applications. Strong communication, organizational, and customer service skills.
