Responsibilities Provide outpatient rehabilitation services in an in-home setting for the older adult population. The setting may be a patient’s private residence or a room in an independent living facility. Deliver therapy programs to facilitate rehabilitation and promote independence. Assess needs, develop, and deliver individualized therapy programs; identify the need for referrals to other disciplines to address comprehensive patient needs. Implement therapy plans designed to restore, reinforce, and enhance performance; educate patients and caregivers to promote independence. Direct activities to promote and maintain health and provide a flexible schedule and work environment across various settings. Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel as applicable and in accordance with state licensure requirements. Adhere to department and patient treatment schedules; maintain timely clinical and billing documentation; participate in related meetings. Attend and contribute to patient care, staffing conferences, and other related meetings. Make referrals to other disciplines and agencies to address comprehensive patient needs. Consult with rehabilitation team members to select appropriate therapeutic regimens aligned with patient needs and capabilities. Adapt programs and plans of care based on individual patient needs. Promote a safe environment and enforce the elimination of fire and safety hazards. Order supplies and equipment as necessary; maintain and report the need for equipment repair. Prioritize patient service to ensure high-quality care in a compassionate setting, respecting patient rights and needs. Screen, examine, and evaluate patients, including history and systems review; perform tests and measures; establish diagnoses and treatment goals; plan and document care; deliver services such as therapeutic techniques, functional training, manual therapy, airway clearance, and wellness education. Design, fabricate, fit, and/or apply adaptive devices including orthotics and prosthetics; train patients in their use. Select appropriate activities suited to the individual’s capacity, intelligence level, and interests. Instruct patients in mobility skills and evaluate progress, attitude, and rehabilitative potential. Perform routine reexaminations to modify or progress the plan of treatment. Incorporate health and wellness programming into treatment plans. Perform home and community mobility assessments and provide safety recommendations for home and community spaces. Perform other related duties as required. Qualifications Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility. Master’s degree in Physical Therapy, or a Master’s in a Physical Therapy related field plus a Bachelor’s in Physical Therapy. Bachelor’s degree in Physical Therapy plus 5 years of progressively responsible PT experience (alternative pathways may apply per credentials). The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated as the U.S. equivalent.
