LVN Charge Nurse (LVN) – $3000 Sign‑On Bonus STGi is currently seeking a LVN Charge Nurse to provide services at The William R. Courtney Texas State Veterans Home located in Temple, Texas. Available shifts: Day (6am‑6pm) and Night (6pm‑6am). A $3000 sign‑on bonus will be paid to full‑time employees. Job Summary To contribute to the success of the community by providing quality nursing care to residents and coordinate total nursing care through implementing procedures, programs, and staying compliant with STGi policies, state and federal regulations. ESSENTIAL FUNCTIONS Follow established standards of nursing practices and implement STGi programs, policies and procedures. Supervise and evaluate all direct resident care and initiate corrective action as necessary. Obtain report from nurse being relieved and record sufficient information to implement appropriate follow‑up action. Provide report to nurse coming on duty, including sufficient information for follow‑up action. Take appropriate action to provide staffing to meet residents’ needs. Conduct resident rounds daily, reporting problems to nursing supervisor and initiating corrective action. Identify safety hazards and initiate corrective action immediately. Identify resident problems and emergency situations and initiate emergency care per physician’s orders in the absence of a physician. Provide supervision to Nursing Assistants providing direct resident care. Attend staff meetings, interdisciplinary team meetings, rehabilitation meetings, in‑service classes and committee meetings as assigned. Review the Glucometer Control Log to monitor that the machine was checked that day prior to testing for blood sugar levels. Follow staffing guidelines and make nursing assignments according to qualifications of staff and characteristics of the resident population. Counsel Nursing Assistants and recommend disciplinary action to the Director of Nursing Service when necessary. Comply with community processes related to pain assessment/management/documentation, providing treatments/documentation and delivering medications/documentation. Administer and document direct resident care, medications and treatments per physicians’ orders and accurately record all care provided. Provide instruction for appropriate nursing care to licensed nurses and Nursing Assistants. Document assessment of residents as assigned per your licensure. Report untoward findings to physician and your supervisor. Observe the 5 Rights of medication/treatment administration. Promptly report changes in residents’ conditions to the physician, Director of Nursing Service and responsible party, and take follow‑up action as necessary. Document notifications in resident’s clinical record. Follow community admission procedures and communicate resident admission to other departments, including rehabilitation. Competently perform basic nursing skills as per your licensure. Follow community discharge/transfer procedure/documentation and coordinate discharge/transfer with other departments, physicians and ancillary service providers. Supervise response to resident’s call for assistance. Listen to resident, family, or physician concerns and initiate corrective action and/or report concerns to Nursing Supervisor. Count narcotics, check emergency kit and exchange keys each shift. Check meal trays and nourishment to monitor proper diet served and residents assisted as necessary. Order, receive and store medications appropriately. Observe community process related to signing out narcotic and administration thereof. This includes the process related to shift narcotic count and documentation. Notify physician, when necessary, of changes in residents’ conditions, weight, medication errors, medication not available, abnormal laboratory/X‑Ray results and any other unusual observations. Document daily notes on unstable residents, change in resident condition, Medicare residents and recently admitted residents. Be responsible for the safety of residents and team members under his/her supervision. Observe infection control procedures. Perform all the duties of a certified nursing assistant when required. Required Experience & Skills Current and valid LVN nursing license in the state of Texas, or a current and valid LVN/LPN license from a Compact Party State. Current Basic Life Support (BLS)/Cardiopulmonary Resuscitation (CPR) certification required prior to performing work, preferably from American Heart Association (AHA), or the ability to obtain certification within 30 days of employment. Working Conditions / Physical Requirements Primarily involves standing, walking, twisting, reaching, bending/stooping, pushing and pulling (typical weight ~5 lb., maximum ~30 lb.) and lifting (minimum 5 lb., maximum 100 lb.). Requires repetitive finger movements as well as hearing and talking on the phone and in person. Must continuously deal effectively with stress created by residents, multiple tasks, noises, interruptions, and work cooperatively as part of the health care team while maintaining a pleasant attitude.
