Overview The Home Care Transition Nurse is responsible for assessing, planning, and developing referrals. 5 x 8 hour shifts (9:30 AM to 5:30 PM) – Rotating Weekends and Holidays Great Healthcare benefits, and education reimbursement of $5,250 annually Additional compensation for visits over your caseload This position will travel between Catholic Health's Skilled Nursing Facilities. Job Details Establish and maintain professional relationships with key physician groups and referral sources. Act as Liaison to referral sources on behalf of Catholic Home Care. Identify potential market growth opportunities and program expansion. Serve as a Customer service representative and act in a professional and supportive manner at all times. Requirements Registered Nurse with current licensure in NYS. 2 or more years of home care experience with patient assessments. Customer Service and Marketing skills. Salary Range USD $90,000.00 - USD $105,000.00 /Yr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience.
