Salary $118,376 to $153,889 per year Appointment type Permanent Work schedule Full-time Duties SummaryThis position will function as a member of the Pain and Palliative Care team and serves as a mid-level provider to initiate, modify or discontinue medication therapy and as a consultant for medication therapy management services under the confines of their scope of practice.If you are not a current VA and/or Federal Employee please apply to CBSU-11133454-21-JR.ResponsibilitiesPrescribing (initiating, adjusting, discontinuing) non-controlled medications for established diagnoses, ordering and monitoring labs, and placing appropriate referrals for other testing/services or supplies as outlined per scope of practice. Perform and document medication reconciliation on patients seen in the Pain and Palliative Care Clinics, with a focus on patient's pain medication history, and other medications that may influence or have the potential to interact with the prescribed pain regimen (ex. Anticonvulsants, Antidepressants, Anxiolytics). Queries the Utah Controlled Substances Database (DOPL) as necessary and documents findings in CPRS. Participate in team huddles and interdisciplinary team meetings. Provide real time drug information, dosing, toxicity, drug-drug interactions, etc., to assist providers in making clinical decisions. Perform population management and high-risk patient identification activities. Participation in safety initiatives, research, and quality assurance activities. Provide continuing education to clinic and Pharmacy staff. Training, precepting and mentoring Pharmacy students and residents. Provide education to patients and their families during clinic hours. Manage non-formulary medication requests related to Pain and Palliative care. Provide support to the facility as it relates to the Opioid Safety Initiative and sit as a committee member on the VHASLC Opioid Safety Taskforce. Coordinate with inpatient medical teams and inpatient surgical teams in conjunction with Pain Medicine/Palliative Care services to provide treatment recommendations and to monitor care for consistency and safety. OR Completion of a PGY2 in Pain and/or Palliative Care OR A minimum of three (3) years' experience in Direct Patient Care related to Pain Management (must be recent history within last 5 years)-this will be determined by the selecting managerGrade Determinations: In addition to the basic requirements, candidates must meet one of the following:

1. One (1) year of experience equivalent to the next lower grade level, or
2. Completion of an ACPE-accredited Pharm.D. program.AND

Must have one (1) year of experience equivalent to the GS-12 grade level. Pharmacists assigned to this position must demonstrate the following knowledge, skills and abilities (KSAs):

1. Knowledge of professional pharmacy practice.
2. Ability to communicate orally and in writing to both patients and health care staff.
3. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security.
4. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

Additionally the following KSAs will need to be addressed at the GS-13 level: Pharmacists assigned to this position must demonstrate the following KSAs: a. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. b. Expert understanding of regulatory and quality standards for their program area. c. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. d. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. e. The work requires standing, walking, moderate lifting and carrying of items, and reaching above shoulders.
