Experience: Minimum of 2+ years in Long Term Care or similar setting

What You'll Do
- Effectively evaluate and rehabilitate patients with physical functioning disorders
- Develop and implement individualized care plans following all regulatory and clinical standards
- Perform physical assessments, administer manual therapy, therapeutic exercises, modalities, and wound care
- Monitor patient progress, adjust treatments, and document outcomes accurately
- Provide consultation and education to patients, families, and healthcare team
- Support and supervise rehabilitation aides, students, or assistants when necessary
- Communicate with the healthcare team regarding patient statuses and treatment adjustments
- Maintain comprehensive, compliant documentation in electronic health records
- Participate in care conferences and continuous training sessions
What You Need
- Valid Oklahoma Physical Therapist license
- At least 2 years of relevant clinical experience in LTC or similar environment
- Strong knowledge of APTA standards, state practice acts, and clinical best practices
- Excellent communication and teamwork skills
