Occupational Therapist

Role Overview: Seeking a licensed Occupational Therapist to provide personalized, in-home and community-based care to older adults, enhancing mobility, independence, and quality of life through evidence-based treatments within a flexible, patient-centered model. Key Responsibilities Deliver one-on-one occupational therapy in private residences and senior living communities. Perform evaluations, develop individualized care plans, and implement treatment strategies. Support patient wellness, functional independence, and safety through therapy sessions. Document care effectively in electronic medical records, ensuring compliance. Collaborate with interdisciplinary teams, including PTs, SLPs, and fitness specialists. Educate patients, caregivers, and staff to promote aging in place and safety. Incorporate telehealth and virtual care to extend access and flexibility. Qualifications & Skills Valid OT license in the practice state or eligibility. Degree from an accredited occupational therapy program. Proficiency with computers and electronic health records. Current CPR certification. Physical & Additional Requirements: Ability to perform moderate physical activities, assist with patient transfers, and drive between locations. PPE usage when necessary.
