Responsibilities

• Administer medications and treatments per physician orders under supervision
• Monitor residents’ vital signs and report changes to the supervising nurse
• Assist with activities of daily living and follow individualized care plans
• Document care provided in the electronic health record and communicate with the RN and interdisciplinary team
• Ensure safety, infection control, and adherence to facility policies
• Participate in shift handoffs and respond to resident needs promptly

Requirements

• Active LPN license in New Mexico
• Willingness to work 12-hour night shifts (3x12)
• Local candidate preferred; able to work within a local travel assignment
• Experience in long-term care or similar setting preferred
• Strong communication and teamwork skills; ability to follow care plans
