Admissions Nurse LPN/RN 10a-6p

Job Summary:

We are seeking a compassionate and detail-oriented Admissions Nurse to join our healthcare team. The Admissions Nurse plays a critical role in evaluating, admitting, and onboarding patients into our facility or service. This position involves conducting initial patient assessments, coordinating care plans, and ensuring a smooth transition into services while maintaining high standards of patient care and compliance.


Key Responsibilities:

* Conduct comprehensive initial assessments of patients to determine eligibility and care needs

* Review medical records, physician orders, and referral documentation

* Collaborate with physicians, case managers, and interdisciplinary teams to develop appropriate care plans

* Educate patients and families about services, procedures, and expectations

* Ensure all admission documentation is completed accurately and in a timely manner

* Verify insurance coverage and assist with authorization processes as needed

* Maintain compliance with all regulatory, legal, and organizational standards

* Provide compassionate support to patients and families during the admissions process

* Participate in on-call rotation if required

Qualifications:

* Current LPN or RN license in North Carolina

* Experience in admissions, case management, or home health preferred)

* Strong assessment, critical thinking, and communication skills

* Ability to work independently and collaboratively in a fast-paced environment

* Knowledge of healthcare regulations and insurance processes

* Proficiency in electronic health records (EHR) systems

Preferred Qualifications:

* Experience in hospice, home health, or acute care settings

* Case management or utilization review experience

* Bilingual skills (if applicable)
