Pediatric Hospice & Palliative Care Registered Nurse Case ManagerUnited Hospice is seeking a compassionate, experienced Pediatric Hospice & Palliative Care Registered Nurse Case Manager to join our growing community-based pediatric hospice and palliative care program.At United Hospice, we believe every child deserves to be comfortable, every family deserves to be supported, and every moment matters. This position offers the opportunity to build meaningful relationships with families, practice nursing with purpose, work alongside a dedicated interdisciplinary team, and make a lasting difference in the lives of children living with serious illness.In this role, the nurse will manage an assigned pediatric caseload in the child's home and community, coordinate an interdisciplinary plan of care, provide skilled nursing assessment and symptom management, educate and support caregivers, and collaborate closely with physicians, nurse practitioners, social workers, chaplains, aides, therapists, and other members of the care team.ResponsibilitiesProvide comprehensive, skilled nursing care to pediatric hospice and palliative care patients in their homes and community settings.Perform initial, comprehensive, and ongoing nursing assessments of the child's physical, developmental, emotional, psychosocial, and spiritual needs.Develop, implement, evaluate, and update individualized plans of care in collaboration with the child, family, attending provider, hospice/palliative medical team, and interdisciplinary team.Assess and manage pain and other complex symptoms, including respiratory distress, agitation, nausea, feeding difficulties, seizures, anxiety, and other symptoms associated with serious illness.Recognize changes in clinical condition and respond appropriately, including coordination of urgent and emergent interventions when indicated.Administer medications and treatments and educate caregivers regarding medication administration, equipment, treatments, and symptom management.Provide skilled nursing procedures appropriate to the child's condition and plan of care.Assist families in understanding disease progression, goals of care, treatment options, and what to expect as the child's condition changes.Provide compassionate end-of-life care and support families through the dying process.Participate in advance care planning and goals-of-care discussions within the scope of nursing practice and organizational protocols.Establish trusting relationships with children and families through compassionate, culturally sensitive, trauma-informed care.Provide education and emotional support to parents, guardians, siblings, and caregivers.Collaborate with the interdisciplinary team to ensure continuity and consistency of care.Participate in interdisciplinary group meetings and case conferences.Coordinate admissions, hospital discharges, transitions of care, and changes in level of care.Coordinate medical equipment, medications, supplies, therapies, and other services necessary to safely care for the child at home.As needed or assigned, make visits and provide care to adult hospice or palliative patients in our community program.RequiredCurrent, unrestricted Registered Nurse license in New York State.Graduate of an accredited nursing program; BSN preferred.Minimum of 2 years of professional pediatric nursing experience.Strong clinical assessment and critical-thinking skills.Excellent communication, organization, and care-coordination abilities.Valid driver's license, reliable transportation, and ability to travel throughout the community.Ability to work independently while functioning effectively as part of an interdisciplinary team.Commitment to family-centered, compassionate, and culturally responsive care.PreferredPediatric hospice or palliative care experience.Experience caring for children with complex medical needs, developmental disabilities, congenital conditions, neurologic disorders, or technology dependence.Hospice nursing experience.Certification in pediatric nursing, hospice and palliative nursing, or a related specialty.Experience with pain and symptom management and end-of-life care.United Hospice Is An Equal Opportunity Employer
