Job Description Knowledge of advanced directives and ability to assist patients and families with completion when needed. Ability to work independently and as part of a multidisciplinary team. Knowledge and skills to screen patients for palliative care needs. Ensure effective communication with patients, caregivers, families, and all disciplines to facilitate a successful transition across the continuum of care. Maintain confidentiality and rights of all patient and departmental documents. Effectively communicate and teach stages of chronic conditions. Effectively communicate end‑of‑life care and make referrals to hospice care when needed. Qualifications Current licensure as a registered nurse in the State of Maryland. At least two years’ experience in a hospital setting. Preferred to have a BSN or other related bachelor‑level degree. Certification in palliative care not mandatory to start, but requested to be completed within two years of employment. Authentic belief and acceptance of palliative care principles.
