Licensed Practical Nurse (LPN) - Behavioral Health Specialist - Conduit Health Partners

Overview The Behavioral Health Access Specialist coordinates the acceptance and throughput of behavioral health patients meeting criteria for inpatient admission from one facility to another in a call center environment. This role performs functions based on defined standards of performance and practice to meet or exceed operational expectations related to transfer center/access center services. The position works collaboratively with internal and external partners to facilitate patient access according to mental health needs and fosters relationships with referring and receiving facility physicians and employees as well as community agencies in representation of Conduit Health Partners' Mission and Values. Responsibilities Coordinate destination placement for mental health patients meeting criteria for inpatient admission from potential referral sources (hospitals, clinics, extended care facilities, physician offices) to initiate acceptance and expedite decision making. Proficiently perform EHR data entry and use telephony/office systems to support operations while protecting PHI; process care level determinations using critical thinking and clinical knowledge to facilitate placement. Provide problem-solving recommendations to optimize patient outcomes based on clinical information, patient/physician preferences, and facility capacity; communicate an accurate clinical presentation to consulting or accepting providers and caregivers. Act as liaison between patients, client facilities, physicians, hospital staff, and outside agencies using professional communication. Demonstrate knowledge of regulatory components (EMTALA, Ethics and Compliance, quality initiatives, HIPAA). Collaborate with providers at client facilities to ensure timely acceptance and access to appropriate level of care/bed assignment using applicable workflows. Identify and report risks related to safety, compliance, and operational inefficiencies via the defined chain of command; offer recommendations for resolution or improvement. Participate in process improvement, professional development, and peer review. Perform other tasks and analysis as requested/directed. Licensing/Certification Active multistate/compact registered nurse (LPN) licensure – Required (compact licensure applicable based on state of residence; if unable to obtain compact licensure, must have active Licensed Practical Nurse (LPN) license) or Licensed Social Worker (LSW) – Required. Education Vocational Training, Licensed Practical Nurse (LPN) (required) or Bachelors degree, Social Work (required). Work Experience 1 year of mental health intake (required). Healthcare contact center experience (preferred). Training Epic electronic health record (preferred). Genesys contact center (preferred). IT Requirements Minimum internet speed of primary and secondary work locations: Download speed 100 Mbps; Upload speed 20 Mbps. Work Environment and Schedule There are opportunities across all shifts (days, evenings, nights) with full-time and part-time positions. Schedules are set and follow a six-week rotation, including an every-other-weekend requirement. There is also an on-call component and a rotating holiday requirement for all positions. Once placed into a schedule, employees are expected to work that set schedule for at least six months before any schedule change request is considered. Schedule change requests are reviewed every six weeks and are evaluated based on established standard operating procedures. During orientation, you will follow the schedule of your assigned preceptor; leadership will attempt to align you with your designated shift, but final assignments depend on preceptor availability. Hours subject to change based on operational need.
