Description

• Conduct evaluations of patients in both inpatient and outpatient settings, developing tailored treatment plans
• Provide physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
• Collaborate effectively with nurses, physicians, case managers, and other rehabilitation professionals to achieve optimal patient outcomes
• Educate patients and their families regarding therapeutic objectives, planning for discharge, and home exercise regimens
• Maintain thorough documentation of evaluations, therapies provided, and patient progress per facility and regulatory standards
• Engage in care team discussions and interdisciplinary conferences to ensure cohesive and streamlined patient care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification is preferred
• Specialty certifications (such as in neurological or geriatric therapy) are advantageous but not essential
