Care Plan Coordinator Job Responsibilities and Duties: Conduct comprehensive assessments and evaluations of patient care needs, ensuring compliance with guidelines. Develop and implement individualized care plans in collaboration with interdisciplinary teams. Perform utilization reviews to ensure appropriate resource allocation and adherence to Medicare regulations. Maintain accurate medical documentation and records in EHR systems, ensuring HIPAA compliance. Provide guidance and support to nursing staff regarding patient care and documentation practices.
