We are actively seeking a Physical Therapist (PT) to join a Skilled Nursing Facility in Hartford, Alabama for a 13-week contract assignment. This is an excellent opportunity to work with a collaborative rehabilitation team while helping patients improve mobility, strength, and overall functional independence.

Key Responsibilities:

• Evaluate patients and develop individualized treatment plans based on clinical findings and functional goals

• Provide skilled physical therapy interventions to improve mobility, strength, balance, endurance, and overall function

• Monitor patient progress and modify treatment plans as needed

• Educate patients, families, and caregivers on exercises, mobility techniques, and injury prevention strategies

• Maintain accurate and timely documentation in accordance with facility and regulatory requirements

• Collaborate with physicians, nursing staff, and interdisciplinary team members to support optimal patient outcomes

Requirements

Qualifications:

• Active Physical Therapist (PT) License

• Graduate of an accredited Physical Therapy program

• SNF experience preferred but not required

• Strong communication, clinical, and patient care skills
