Primary Care Physician

Job OpportunityPhysicians Alliance of Connecticut (PACT, LLC) has an Internal Medical Group with an opportunity for a physician to work in skilled nursing facilities in the Waterford area. This is an excellent opportunity for those that enjoy a flexible work schedule, working autonomously, and accelerate in a pay for performance environment. The position would be full time working 40 hours per week.Under indirect supervision, provides and manages health care for a specified patient population.Essential functions include:Providing and managing direct patient care, including physical examinations, evaluations, assessments, diagnoses and treatment for a specified patient population.Engaging patients and their families or caregivers in understanding, setting and monitoring self management care plans, and documenting individualized care plans and care coordination in the patient chart.Analyzing patients medical history, medication allergies, physical condition and examination to determine best care.Diagnosing medical conditions.Prescribing pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions.Referring patients to specialists or to internal medicine as appropriate.Directing and coordinating the patient care activities of nursing and support staff as required.Respecting and promoting patient rights and privacy.Responding appropriately to emergency situations.Participating in meetings to evaluate practice processes and work flows pertaining to patient care and quality improvement efforts.Participating in specified health promotion, education and/or prevention programs.Participating in population health activities as directed by the practice.Following established departmental policies, procedures, and objectives, continuous quality improvement objectives, and safety, environmental, and/or infection control standards.Skills and knowledge required include:Knowledge of legal and ethical standards for the delivery of medical care.Understanding of relevant drugs and non-pharmaceutical patient care aids and ability to prescribe dosage and instruct patients in correct usage.Knowledge of current principles, methods and procedures for the delivery of medical evaluations, diagnosis and treatments in area of expertise.Demonstrated skill in interviewing and patient coaching.EHR documentation as it pertains to patients and populations of patients.Education and experience required:Medical Doctor with 3 to 5 years of direct patient experience which may include residency.Medical License, Board certification, Federal DEA certificationPreferred behaviors:Detail Oriented: Capable of carrying out a given task with all details necessary to get the task done well.Dedicated: Devoted to a task or purpose with loyalty or integrity.Preferred motivation:Ability to Make an Impact: Inspired to perform well by the ability to contribute to the success of a project or the organization.Equal Opportunity Employer This employer is required to notify all applicants of their rights pursuant to federal employment laws.
