Admissions Nurse
Boston Medical Center
Gilchrist Hospice Care seeks a Hospice Admissions Nurse to assess patients in the home or hospital, then prepare admission into the hospice program. You will collaborate with the Medical Director and care team to implement an initial plan and provide direct nursing care. You will educate patients and families on care procedures, medications, and device use, while coordinating with outside agencies to meet needs and ensure a smooth transition. #J-18808-Ljbffr
