Physical Therapist (PT)Brookhaven Health Care Facility is looking to add a Physical Therapist (PT) to its rehabilitation team. Join a team where collaboration, respect, and meaningful work are at the heart of what we do.Responsibilities:Evaluate residents and develop individualized physical therapy treatment plansEstablish short- and long-term rehabilitation goals and monitor resident progressProvide skilled therapy services to improve mobility, strength, balance, endurance, and functional independenceSupervise Physical Therapist Assistants (PTAs) in accordance with New York State regulationsComplete required documentation, evaluations, care plans, and billing records accurately and timelyCollaborate with physicians, nursing staff, families, and interdisciplinary team members to support resident outcomesProvide resident and family education regarding therapy goals, adaptive equipment, and home exercise programsRecommend and coordinate durable medical equipment (DME) and discharge planning needsParticipate in care conferences, staff education, orientation, and quality improvement initiativesMaintain therapy equipment, supplies, and treatment areasPerform additional duties as assignedRequirements:Master's or Doctorate Degree in Physical Therapy from an accredited program approved by the American Physical Therapy AssociationValid New York State Physical Therapist License requiredNewly licensed Physical Therapists encouraged to applySkilled nursing facility experience preferredStrong clinical, documentation, communication, and problem-solving skillsAbility to work collaboratively within an interdisciplinary healthcare teamCommitment to providing compassionate, resident-centered careBenefits:Paid Time Off (PTO) for Full-Time and Part-TimeWeekly or Same-Day Pay OptionsHealth, Dental, and Vision InsuranceLife InsuranceEmployee Referral Bonus ProgramRetirement Saving OptionsEqual Opportunity Employer/Protected Veterans/Individuals with Disabilities This employer is required to notify all applicants of their rights pursuant to federal employment laws.
