Responsibilities

• Perform comprehensive physical therapy evaluations for residents in a skilled care setting
• Develop and implement individualized treatment plans focusing on mobility, gait, balance, strength, and range of motion
• Monitor progress, document outcomes, and adjust plans as needed
• Provide gait and transfer training, fall prevention strategies, and ROM exercises
• Educate residents and families on home exercise programs and safety measures
• Collaborate with an interdisciplinary team and participate in care planning discussions

Requirements

• Active Physical Therapist license or eligibility for licensure in the appropriate state
• Doctor of Physical Therapy (DPT) degree or equivalent
• Experience or demonstrated interest in geriatric rehabilitation or skilled nursing facility settings
• Proficiency with electronic medical records and clinical documentation
• Ability to work effectively in a skilled nursing environment and follow established care plans
• Strong communication and teamwork skills
