Community Transition Case Manager

Community Transition Case Manager

Salary: $43.57/hr.-$55.89/hr.

Essential Functions:

  • Oversees case management for guests of Community Transition Program.
  • Conducts initial assessments/intakes to identify a new guest's current resources and needs and to identify possible barriers to attaining their goals.
  • Researches guest legal history and identifies classifications of crimes that may exclude a new guest from the shelter.
  • Coordinates monthly case management team meetings for Community Transition Program guests.
  • Assists guest in setting goals and identifying steps towards meeting those goals, including referrals to resources, and accessibility.
  • Meets regularly with guests on at least a monthly basis, more with guests needing intensive case management.
  • Assists guests with applications for services, using service plans to prioritize needs.
  • Maintains well organized and up-to-date case files and a database that tracks guest information and outcomes.
  • Provides records and reports as requested by Health & Wellness Leadership.
  • Assists in the preparation of summary reports of activities and operations.
  • Assists guests with coordination of their transportation needs through local community resources.
  • Transports guests in an agency vehicle when required.
  • Represents Community Transitions Programs at meetings and events in the greater community.
  • Oversees monthly inspections of each home. Work with guests to maintain safe, clean, and presentable homes, community building, and outside area.
  • Expands and maintains connections with various area resources including mental health, chemical dependency, employment specialists, etc.
  • Monitors guest's risk of harm to self, others and property, and refers to appropriate resources.
  • Utilizes crisis intervention techniques to maintain individual and community stability.
  • Investigates complaints and reported incidents as needed.
  • Responds to security and police reports of suspicious activities – on call 24 hours/7 days a week.
  • Maintains composure in adverse conditions.
  • Participates in trainings and other professional development opportunities as needed.
  • Performs a variety of other duties as assigned by supervisor.

Supervisory:

  • Ensures the hiring, training, promoting, assigning, disciplining and discharging of employees are in accordance with Tribal Law, including without limitation, the Tribal Employment Rights Ordinance (TERO).
  • Assures staff adherence to all policies and procedures, work, safety and administrative rule, Tribal Law and State Regulations.
  • Serves as a mentor, as requested, for Tribal career development.

Education, Experience, and Licenses:

  • Requires a Bachelor Degree in Social Services, Community Health or a related field.
  • Requires five (5) years' directly related work experience in housing operations.
  • Requires two (2) years providing direct supervision to subordinate staff.
  • OR equivalent combination of education, training and experience.

Driving Requirements:

  • Employees may be required to drive for work-related purposes and must maintain a valid Oregon Driver's License (ODL), meet company vehicle insurance requirements, and provide proof of personal vehicle insurance.

Qualifications, Registrations, and Certifications:

  • Must have experience with affordable housing, homelessness outreach and engagement strategies, and chronic and mental health issues.
  • Must have working knowledge of housing navigation, housing and landlord/tenant rights, public benefits applications, and eviction prevention.
  • Ability to work effectively with diverse clients with a broad spectrum of assets and needs, particularly mental health, substance use disorder, and life skills issues.
  • Excellent interpersonal skills, including written and oral communication.
  • Ability to work both independently and as a team player; willingness and ability to make sound judgments without onsite supervision.
  • Ability to work with high-need individuals while maintaining healthy boundaries.
  • Working knowledge of local human services, employment, housing, mental health and chemical dependency services.
  • Ability to recognize a crisis and provide crisis intervention.
  • Must have the ability and experience to act quickly and calmly under emergency situations.
  • Ability to communicate and work effectively with diverse populations and cultural differences, including staff, volunteers, guests, students and board members.
  • Commitment to social justice for people living in poverty, and a thoughtful approach to strategies for achieving it.
  • Must be organized and detailed: ability to manage multiple cases efficiently and effectively.
  • Must have passion for ending homelessness.
  • Ability to work as a member of a team and independently.
  • Requires working knowledge and proficiency in use of office equipment and computer software such as Microsoft Word Programs.
  • If a recovering chemically dependent person must be in a program of recovery and able to provide documentation of sobriety for a minimum of two (2) years.
  • Knowledge of and sensitivity to Native American culture required.

Environmental Factors and Conditions/Physical Requirements:

  • Continually requires the ability to express or receive detailed information or important instructions through oral communication.
  • Continually requires working with fingers rather than the whole hand or arm. Requires repetitive movement of wrists, hands and/or fingers. Often requires walking or moving about to accomplish tasks.
  • Requires standing and/or sitting for sustained periods of time, raising objects from a lower to a higher position and/or moving objects horizontally.
  • Continually requires color identification, close vision, far vision, and depth perception.
  • Occasionally requires lifting of up to 40 pounds.
  • This position is frequently subject to inside environmental conditions which provide protection from weather conditions, but not necessarily from temperature changes, and is occasionally subject to outside environmental conditions.

Supports a safe work environment by performing all duties in accordance with Tribal safety policies and procedures, and Emergency Management Plan.

Due to the nature of the interactions with other employees and the public we serve, employees may be required to provide documentation of, or receive, certain health vaccinations.

May be called upon to serve as a Disaster Reservist during emergencies.

Local travel is occasionally required. Travel outside of the state is infrequently required.
