One (1) to three (3) years’ experience in a long‑term care community required; or equivalent combination of education and experience. Knowledge of long‑term care practices. Compassion for older adults, practicing with a person‑centered approach that exhibits a commitment to hospitality and excellence in personalized assistance. Current active status or ability to register as a Certified Dementia Practitioner within the first 90 days of hire through the National Certification Council for Dementia Practitioners (NCCDP). Job Summary The Social Worker is primarily responsible for providing services to residents in the Independent Living. This position is based on the principles of a strengths‑based, person‑centered approach that recognizes and tailors interactions to a person’s best abilities, supporting their overall well‑being through providing meaningful social interaction, personal care and other tasks as designated. If the jurisdiction requires and the individual has the credentials, independent living social worker will provide social services “supervision/oversight” for our health center staff. The Social Worker is responsible for providing exceptional customer service, utilizing courtesy, dignity, and respect in all of the following responsibilities: Responsibilities Meets with visitors on a need be basis. Responds to telephone and fax contacts. Meets with family to assure that the admission transition runs smoothly; assists with any additional questions. Aids residents and families through individual or group conferences to understand, accept, and follow medical recommendations. Enter resident information at admission into Minimum Data Set at 5, 14, 30, 60, 90 days as well as quarterly. Complete care plan reports focusing on behaviors and psychosocial needs and send to MDS Coordinator. Complete all necessary social services paperwork in a fast and efficient manner. Ensure all information on the resident is accurate. Coordinate with staff to ensure psychosocial and emotional needs of residents are met and document the same. Grievance Responsibilities Ensure that residents know how to file grievances, implement and manage the written grievance policy to ensure the prompt resolution of grievances. Ensuring and leading prompt response to a grievance and taking required actions. Ensures grievance policy meets requirements and regulations and is being followed. Educates residents on grievance policy. Ensures a review of the policy and plan on an annual basis.
