Home Infusion Staff Pharmacist Opportunity – Syracuse, NY
A direct hire position is available for a knowledgeable and dedicated Home Infusion Staff Pharmacist to provide comprehensive pharmaceutical care and support in the Syracuse, NY area. This role is integral to ensuring safe and effective home infusion therapies while collaborating closely with multidisciplinary healthcare teams.
Schedule:
- Monday through Thursday
- 8:00 AM – 5:00 PM
Key Responsibilities:
- Review and validate infusion orders and patient therapies with precision and clinical insight
- Participate actively in clinical rounds to assess and optimize patient care
- Perform sterile compounding and total parenteral nutrition (TPN) review ensuring accuracy and compliance
- Maintain detailed patient records and individualized care plans
- Uphold USP standards and maintain cleanroom compliance to guarantee safety and quality in compounding processes
- Provide training and ongoing support to pharmacy staff to enhance operational effectiveness
- Drive initiatives to improve pharmacy operations and patient care outcomes
Qualifications:
- Valid licensure as a Registered Pharmacist in the State of New York is required
- Minimum of two years of recent acute care experience, or at least one year in home infusion therapy preferred
- Strong expertise in intravenous admixture services and intravenous therapy
- Proven ability to assess and monitor drug therapy tailored to home care patients in coordination with physicians, nurses, and other healthcare professionals
