Physical Therapist, Per Diem

Responsibilities Evaluate referred patients to assess their medical condition, functional limitations, restrictions, and complaints. Establish and administer a treatment program with specific goals based on the patient’s capacity and tolerance, under the guidance of the Medical Director. Write evaluations for new physical therapy patients. Reassess ongoing patients. Develop and adjust patient treatment plans. Create objective and measurable goals with specific time frames. Provide patient, family, and community education. Deliver rehabilitative physical therapy care to inpatients and outpatients, including modality/exercise treatments, wound care, mobility and gait training, and other therapies. Document progress notes and flow sheets/charts in accordance with department guidelines, Medicare, Worker’s Compensation, or other payors. Write discharge assessment notes. Requirements Bachelor of Science degree from an APTA approved school of Physical Therapy. Valid California Physical Therapist License. Eligibility for membership in the APTA. Adherence to the code of ethics of the APTA. Basic Life Support (BLS) certification from the American Heart Association (AHA). Valid Los Angeles County Fire Card (required within 30 days of employment). Assault Response Competency (ARC) certification (required within 30 days of employment). #J-18808-Ljbffr Jobtailor
