Physical TherapistAssess, plan, organize, and participate in rehabilitative programs that improve mobility, relieve pain, increase strength, and improve or correct disabling conditions resulting from disease or injury.As a Physical Therapist, you will:Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.Apply concepts of infection control/universal precautions in performing patient careAssure continuity of quality patient care delivered with appropriate documentationProvide effective communication to patient/family, team members and other health care professionalsPromote agency philosophy and administrative policiesMaintain and complete all documentation within company stated guidelines.RequirementsSuccessful completion of accredited physical therapy program and state licensure exam.Valid Physical Therapy license1 year of home health experience preferredCPR certificationValid Driver's licenseValid Auto InsuranceHomeCare HomeBase experience preferredPhysical DemandsMust be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.Must be able to lift, carry, pull, or push up to 100 lbs.
