Job Description:

Essential Functions:


- Collect relevant data related to patients' health and situations.

- Analyze assessment data to determine diagnoses and care issues.

- Develop comprehensive care plans with targeted interventions to achieve desired outcomes.

- Coordinate care delivery and promote a safe environment for healing.

- Evaluate progress towards goals and identify outcomes for patients.

- Collaborate with patients, families, and healthcare providers to ensure holistic care.

- Provide valuable learning opportunities for patients and team members.

- Actively participate in discharge planning to maintain continuity of care.

- Delegate tasks and coordinate the duties of the healthcare team effectively.

- Complete additional job-related duties as assigned.

Requirements:


- Bachelor's Degree in Nursing (BSN): Preferred

- Current Registered Nurse (RN) licensure in the state of practice: Required

- Certification in Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS): Required

- AHA-BLS or ARC-BLS: Required

- AHA-ACLS or ARC-ALS: Required

- AHA-PALS or ARC-PALS: Required

- National Institutes of Health Stroke Scale: Required

- At least two years of recent experience in the field.
