Position: Registered Nurse - Home Health

Shift:

Day shift; 5 days per week, 8 hours per day (08:00 AM - 05:00 PM)

Job Description:

This role is responsible for coordinating and directing the provision of home nursing care according to physicians' orders. The successful candidate will follow agency policies and procedures while applying the nursing process competently. The position reports to the Clinical Manager and includes supervising and evaluating Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.

Key Responsibilities:

• Provide skilled nursing care to patients in their homes based on individualized care plans.
• Evaluate patient needs and adjust care plans accordingly.
• Coordinate with physicians and other healthcare professionals.
• Supervise and support Home Health Aides and LPNs/LVNs in delivering quality care.
• Complete necessary documentation in compliance with agency standards.

Requirements:

• Minimum of 1 year of adult home health or hospice experience.
• Current and active nursing license is required.
• Certification in CPR and BLS is necessary.
• Proficiency in HomeCare HomeBase (HCHB) and OASIS is required.
• Reliable transportation and a valid driver's license are mandatory.
• Auto insurance as per agency requirements.

Travel Requirements:

• Must be willing to travel up to 75 miles from the home clinic for patient visits.
• Mileage will be reimbursed at the IRS Standard rate.

Schedule and Productivity Expectations:

• Guaranteed hours are not provided; however, options are available to float to nearby clinics if needed.
• Expected patient load is approximately 5-10 patients per day, aimed at achieving a minimum of 30 points weekly.

Work Environment:

• This position requires visits to patients' homes as well as periodic office meetings.
• The candidate should be comfortable with both home visits and commuting.

Location:

Roseburg, OR

Additional Information:

The role may require participation in a rotating on-call schedule with other RNs. All documentation must be completed within 24 hours post-visit to ensure compliance and efficient follow-up care.
