Nurse Admissions Coordinator

Position Description:

Enhances the quality of life for people with intellectual or developmental disabilities who live in Host Homes by providing leadership and support to the program and staff. Assists the Director of Host and Companion Homes to ensure that people receiving services are supported in creating the lives they desire and that Host Home partnerships are successful. Acts as liaison between individuals receiving services, Host Home Providers, family members, and team members to ensure the pursuit of the individual's goals and choices.


Essential Duties:

1. Ensures that the self-expression, self-worth, self-reliance, and decision making of individuals living in Host Homes is supported and respected through Host Home services.
   • Models quality and professional practices and communications.
   • Works in capacity of Host Home Care Coordinator and Nurse with a caseload of individuals living in Host Homes that fluctuates based on departmental needs.
   • Works in capacity of Admissions Coordinator to ensure that individuals accessing Host Home Services are admitted in a fashion that promotes quality and individuality.
   • Fosters collaboration with individuals receiving services, their loved ones, the Host Home providers, and their support team.
   • Ensures that training, resources, and work environment are provided to support individuals' receiving services in pursuing their goals, activities of interest, and social interactions.
   • Ensures implementation of the Annual Plans and Individual Service and Support Plans (ISSPs) for people receiving services.
   • Ensures development and implementation of Care Plans specific to the medical needs of the people receiving services.
   • Working closely with the Director, continually evaluates and assesses the programs' strengths and weaknesses to improve services and supports using a person-centered approach.
2. Supports a culture where individuals receiving services are healthy, safe, and valued by others.
   • Models and teaches respect for others, including individuals in services, staff, Host Home Providers, guardians and loved ones, and team members.
   • Ensures the safety and well-being of individuals receiving services by providing oversight, guidance, and resources to Host Home Providers.
   • Collaborates with and assist PPCH staff and community stakeholders to carry out departmental and PPCH goals and objectives.
   • Ensures that the preferences of individuals living in Host Homes are considered, and that homes are clean and maintained according to standards.
   • Conducts required assessments of individuals receiving services and ensure prompt follow-up when deficiencies or areas for improvement are found.
3. Responsibility for compliance with PPCH policies, and the rules and regulations of State and Federal oversight agencies.
   • Working closely with Director and Quality Assurance, in complying with documentation requirements
   • Ensures bookkeeping and recordkeeping for caseload meet the standards of PPCH and State and Federal agencies.
   • Oversight of and responsibility for current and complete Host Home tracking and documentation for caseload.
   • Ensures Host Home Providers and Respite Providers on caseload have completed required trainings, including annual refreshers, according to established timelines.
   • Maintains knowledge and understanding of PPCH and other regulating agencies' policies, procedures, rules, regulations, and guidelines.
4. Ensures that the self-expression, self-worth, self-reliance, independence, and decision-making of individuals receiving services from PPCH is supported and respected.
   • Assists with the onboarding and training of new contracted providers, including customer-specific training (CST).
   • Responsible for the daily operation of a caseload for people who receive services across PPCH programs for the first 30-90 days, as needed.
   • Maintain a host home caseload when necessary.
   • Provides support and services to new admissions on their caseload, including:
     • Assisting with onboarding of new people receiving services and host home providers
     • Documentation training for new providers
     • Creation of the required PPCH Client Folder, including:
       • Consent packets
       • HRC, if needed
       • PARs and Service Plans
       • IDT attendance
       • CST
       • Host home visits and audits
       • Care plan development and implementation
       • MAR development and maintenance
       • Process physician orders
   • Ensures that training, resources, and work environment are provided to support individuals receiving services in pursuing their goals, activities of interest, and social interactions.
   • Attends team meetings and ensures implementation of the person-centered service plan and applicable rights modifications for people receiving services.
   • Works closely with the directors of PPCH programs to ensure a smooth transition into services.
   • Evaluates and assesses the programs' strengths and weaknesses to improve services and supports using a person-centered approach.

Other Duties:

1. Responds to requests from internal and external auditors.
2. Attend meetings and staffing as needed or requested.
3. May assist Host Home Care Coordinators with Annual Plan or ISSP meetings or preparation.
4. Works in the capacity of administrative or other Residential staff as needed.
5. Other duties as assigned.

Qualifications:

Education and Training:

• High school diploma or equivalent required.
• Bachelor's degree preferred.
• Licensed Practical Nurse/Registered Nurse
• Required training will be provided and must be successfully completed.

Experience:

• 5 years' work experience in the field of intellectual or developmental disabilities required.
• 2 years' work experience as a Host Home Care Coordinator or equivalent.
• One year experience working in the Colorado Developmental Disabilities System required.
• Minimum of one year using Microsoft Office or other software required.

Knowledge, Skills, and Abilities:

• Excellent written and verbal communication skills.
• Effective listening skills, even when non-traditional communication is used.
• Experience with EHRs or Therap
• Ability to lead and manage others.
• Knowledge of Colorado Developmental Disabilities system, community support systems, and human services.
• Knowledge of Social Services and out of home placements for adults with intellectual or developmental disabilities.
• Ability to respectfully address significant issues of health and safety while supporting an individual's choices.
• Ability to work as part of a comprehensive team and represent PPCH in the community.
• Skill and ability to constructively receive and provide feedback.
• Ability and willingness to complete required trainings and learn PPCH operations and systems.
• Valid Colorado driver's license, and ability to meet PPCH's driving requirements.
• Ability and willingness to respond to emergency situations in Host Homes.
• This position requires regular use of a personal vehicle with liability insurance that meets PPCH standards.

Physical Requirements and Working Conditions:

• Work will often be performed in a office environment, using a computer and other typical office equipment. Work may be performed in private homes or in the community using a laptop and/or mobile device.
• Lifting and carrying of approximately 15 lbs.
• This position may occasionally involve physically assisting others, including pushing/pulling individuals using wheelchairs.
• This position is on-call approximately one week every three months.
• Regular and frequent driving throughout the Denver Metro area required.
